Craniotomy

Underlying Problem Requiring A Craniotomy:
Someone suffering from a Meningioma Tumor, brain lesions or other brain trauma may need to undergo a craniotomy in order for a surgeon to gain access to the brain. Additionally, this procedure is used in neuroscience to treat epilepsy, Parkinson's disease, and cerebellar tremors.

Craniotomy Procedure:

Before the Craniotomy takes place, the patient will undergo an MRI, which provides a detailed image of the brain. This allows the Neurosurgeon to know precisely what areas of the brain need to be accessed, and accordingly how much skull must be removed. As you may have guessed, a Craniotomy is a surgical procedure in which part of the skull, called a bone flap, is removed, providing access to the brain. This procedure is usually performed under general anesthesia (in which the patient is completely asleep), but can also be done with local anesthesia (with the patient awake).

Depending on the end goal, a Craniotomy may require that only a small hole is made in the skull, which can heal quickly, or that a large piece of skull be removed. If the latter is required, the Neurosurgeon will replace the piece of skull at the close of surgery and will secure it with metal plates until it is healed. The metal plates are then removed. The surgery can take up to 5 hours, but may even go longer, depending on the complexity of the issue.

There are six steps that a patient will take. First, the patient will prepare for surgery by not eating or drinking past midnight the night before the surgery. The patient will also have a lumbar drain inserted into the lower back to help remove cerebrospinal fluid, allowing the brain to relax during surgery. Second, the first incision is made. The scalp is first cleansed with an antiseptic and the then the Neurosurgeon makes the incision, usually behind the hairline. Third, the skill is opened. Skin and muscles are lifted off the skull and a small hole is made with a drill. Then the Neurosurgeon uses a special saw to cut the desired flap. Fourth, the surgeon folds back the dura to reveal the brain and then begins his work on the affected area for treatment or removal. Fifth, the Neurosurgeon uses special tools to repair or remove the desired portion of the brain. The Neurosurgeon uses magnification glasses to ensure that he or she can see every detail, ensuring that no damage is done to healthy brain tissue. Sixth, the bone flap is replaced and the dura is held together with sutures. The muscles and skin are also sutured back together and the patient begins recovery.

Recovery from a Craniotomy:

The anesthesia will wear off while the patient is in the recovery room. At the point, the ventilator tube is removed from the patient's lungs. Next, the patient is transferred to the neuroscience intensive care unit, where the patient's vital signs will be monitored. This may take a few days to two weeks, depending on the circumstances.
